New versions of the Criminal Defence Service (CDS)
forms 14 and 15 are now available. The forms have been designed
with input from practitioners, with the aim of improving accuracy
in completion and to ensure that all relevant information is
captured. It is hoped that this will help to reduce delays to
processing applications that can cause cases to be adjourned.
The new forms will be available from 1 February 2012. The old
versions of the forms can be used until the LSC informs providers
that stocks have been depleted, which is anticipated to be by the
end of February.
