How do I request a client’s medical records?

I need to access my client’s GP and hospital health records to proceed with a case. I understand there's a Law Society and BMA-approved form to request the records. Where can I get a copy?

You can download a copy of the consent form, which has been agreed with the British Medical Association (BMA), from our website.

The form includes useful guidance notes for clients, solicitors and healthcare record controllers.

It’s aimed at improving protection for the public when they agree to release their health records.

The form will demonstrate that the patient’s informed consent has been gained before health records are disclosed.
